CPCS is licensed by the state of Texas and therefore the policies set by the state must be followed. This includes a variety of paperwork. All paperwork must be
completed before a child may attend school.
- Enrollment form – A completed Enrollment form must be in the school office upon
enrollment. CPCS also requires each family to complete an “Emergency Card” each year. The card updates pertinent information that is needed by the school. It is the responsibility of the parents to inform the school of any changes in phone numbers, emergency contact information or medical information.
This is necessary for the safety of all the children. The numbers on the emergency card are the ones used to notify a parent in the event of an illness, accident,
or any other emergency.
- Health Form – A completed health form must be in the school office upon enrollment. The
form must be signed by the child’s physician indicating they are able to participate in school activities. An updated immunization record must also be included. Exemptions from immunizations must
meet criteria specified by the Texas Department of Health. Information concerning exemptions can be found on the Department of Health website at https://www.dshs.texas.gov/immunize/school/school-requirements.aspx. Parents must inform the school of any additional immunizations received after the form has been given to the school. Children who do not have a medical form on file will not be
permitted to attend the program. (At this time, Collin County does not require Tuberculin testing.)
- Hearing and vision screening must be done for
children enrolled in our four years old or TK progam for possible vision and hearing problems. Crossing Point Christian School provides this screening for all children enrolled. There may be a charge
for this service. Parents may have their children screened by their own physician. A copy of the results of the screening must be given to the school by the end of the first semester.
The school will notify parents immediately of any policy or procedural changes made by the school or the state of Texas.
